We want to make sure Crossroads is the right fit for your child before scheduling an evaluation. Our front office team will guide you through each step of the process.
1. Contact Us
Start by completing our online contact form or calling our office. A member of our front office team will follow up with you to learn more about your child, including their age, diagnosis or primary concerns, and the type of services you are seeking.
2. Insurance & Availability
Before scheduling an evaluation, we will review current therapy availability and verify your insurance benefits when applicable. Our team will provide information about your anticipated coverage and financial responsibility so you know what to expect before moving forward.
Because our goal is continuity of care, we generally schedule evaluations when we anticipate having an appropriate therapy opening available if treatment is recommended.
3. Complete Intake Forms
Once we determine that we have an appropriate opening, your information will be entered into our secure electronic medical record system. You will receive a text with access to the intake and case history forms that should be completed before your evaluation.
4. Prepare for Your Evaluation
Please bring copies of any relevant previous evaluations, medical reports, or other records that may help us better understand your child's history.
For feeding evaluations, your therapist may also ask you to bring specific foods, bottles, cups, utensils, or other feeding items your child regularly uses.
5. Evaluation & Recommendations
Your therapist will complete a comprehensive evaluation based on your child's individual needs. Following the evaluation, we will discuss findings with you and make recommendations regarding therapy, including appropriate frequency and duration when treatment is indicated.
6. Begin Therapy
If therapy is recommended and you choose to move forward, our team will work with you to establish a treatment schedule and begin your child's individualized plan of care.
